A multi-agency Live Well Hub event was held at Martock Parish Hall on Friday 22nd May, bringing together health, social care, and voluntary sector partners to help improve access to support for local residents.
South Somerset West PCN were on hand to support on the day providing mini health checks and overall advice on all aspects of the support and services that we provide and have links to.
The event focused on delivering Care advice and Carer’s Assessments (targeting those on waiting lists), providing adult social care advice and information & offering a drop-in opportunity for the wider community to learn about available services.
Attendees benefited from mini health checks from Symphony Healthcare Services, support from Somerset Council Adult Social Care teams,
input from District Nurses (Somerset FT) and wider community health professionals, & advice from partner organisations including NHS Somerset, Open Mental Health, Thrive, and MIND.
