Great to attend the Neighbourhood Breakfast hosted by Mind in Yeovil and connect with so many organisations making a real difference across our communities.
With around 80 people attending, representing a wide range of voluntary, community and health sector organisations, the event showcased the breadth of support available locally and the passion of the organisations dedicated to improving people's lives every day. It was also an opportunity to share work taking place across South Somerset West Primary Care Network to help reduce hospital admissions through proactive, community-based support.
Working closely with voluntary and community partners helps ensure people can access the right support, at the right time, in the right place.
