
Somerset NHS Foundation Trust (SFT) recently held their ‘Our Somerset Colleague Awards’ (OSCAs) – an evening all about celebrating the incredible people behind our NHS services.
We are delighted to say that the The Partnership Award was won by the Peri-Operative Service!
The Peri-Op team looks after patients from the moment surgery is planned through to recovery.
The team has changed the way patients get ready for surgery by setting up local outreach clinics. Instead of a hospital-based pathway, these clinics bring together GPs, community services and other support in one place, closer to where patients live.
Somerset’s first clinic, was held in collaboration with Symphony South West (SSW) Primary Care Network (PCN), at Martock Surgery.
Patients can have their health checked and talk about things that could affect their surgery, such as smoking, alcohol use, weight, fitness, social support and emotional wellbeing.
Together with partners, the team helps create a personal plan, so each patient is as ready as possible for their operation.
Congratulations to all and well deserved. A great example of partnership working for the benefit of patients!
