We are adapting the way that healthcare services are offered in Primary Care to meet the changing needs of our patients, both for today and the future; providing them with the care and support they need to live healthy, independent lives.
Within the practices who choose to join SHS; patients will begin to see an extended healthcare team who work around the GP to support, diagnose and treat both simple and serious healthcare issues. This enables our GPs to provide the complex healthcare support to the patients who need it the most.
Specialist roles such as Musculoskeletal Practitioners (Physiotherapists) and Pharmacists are joining our teams so that patients can access specialist advice and treatment, without needing to be referred to hospital first- reducing waiting times. Our Nurse Practitioner teams can diagnose, treat and prescribe on a wide range of health conditions, alongside our doctors. Health Coaches work as part of every practice team and can offer proactive and preventative support to help you to take greater control over your health and well being. Those with multiple complex long-term healthcare conditions, can access the specialist care and support required to help them manage their health, remain independent and enjoy quality of life at home, avoiding unnecessary hospital admissions.
